Community Health Worker

Vancouver Location

The Registered Care Aide (RCA)/ Community Health Worker (CHW) will provide individualized care that is sensitive to the client’s unique needs, providing assistance with ADLs and IADLs through client specific assignment working under the direction of and in collaboration with a regulated health care professional. Care will take place in the community, within a client’s home.


  • Graduate of a certificate or diploma course from a program approved by the BC Care Aide & Community Health Worker Registry
  • Experience with elderly population is preferred
  • Current CPR and First Aid obtained within Canada
  • Current criminal check with clear Vulnerable Position Screening completed within the last 6 months
  • TB Two Step completed within the last 6 months
  • Open and flexible availability – open evening and weekend considered a major asset
  • Must have a valid driver’s license and own vehicle
  • Must be available to work Monday – Friday 8:00am – 3:00pm
  • Must be willing to drive throughout the Richmond areas


  • Following care plan, assist clients with daily care, mobility and ambulation
  • Provide support to clients and families
  • Communicate with supervisor and Health Care team members regarding changes or concerns related to client
  • Documentation regarding client activities, progress, and treatment plans
  • Adhere to the policies to ensure client/family privacy and confidentiality
