We recently connected with Iris, a home health care Social Worker, whose passion for empowering clients shines through every visit. She shared how the small but powerful moments motivate her each day.
Advocacy and support
“What continues to motivate me is seeing how access to the right support at the right time can truly change someone’s experience at home,” shares Iris.
When patients face barriers to accessing essential resources or services, social workers step in to address these challenges. They may navigate insurance issues, connect clients with community resources, or advocate for additional services that support their overall wellbeing.
“I’ve helped clients by connecting with building management to ensure timely repairs or communicating with landlords regarding pending evictions. Even small interventions can significantly reduce stress and stabilize a client’s living situation,” Iris says.
Assessment and care planning
When social workers enter a patient’s home, they conduct comprehensive assessments to understand each client’s unique needs, strengths and challenges. These assessments consider medical conditions while also exploring factors such as living arrangements, social support systems and emotional wellbeing.
“The first step is listening. Every client’s situation is unique, and understanding their values, environment, family dynamics and personal goals helps guide the care plan,” says Iris.
Based on their findings, social workers collaborate with the care team to ensure the holistic needs of each person are addressed in personalized ways.
“One of the most important considerations is recognizing that not every solution fits every client… creativity and flexibility are essential in community care,” Iris adds.
Family and caregiver support
Family members or caregivers are often actively involved in a patient’s care journey. Social workers recognize the significance of family dynamics and the impact they can have on the patient’s well-being. They can provide education and support to family caregivers so they can most effectively care for their loved ones. Social workers can also help caregivers access respite care services and self-care resources to prevent burnout and support their own wellbeing.
By addressing the complex and often unseen challenges patients face at home, social workers help ensure care extends far beyond medical needs. Their work strengthens connections, empowers families and helps people navigate challenges with dignity and support.
