Connected care at home | How ParaMed is supporting the Guelph Wellington OHT Home Care Program

With its first year of operation now complete, the Guelph Wellington Ontario Health Team (OHT) Integrated Patient Care Team (IPCT) Home Care Program is a testament to the real impact innovative partnerships can have on patients, their families and the local health system.

Through this program, ParaMed works with care coordinators and participating primary care providers to document a patient’s care in one shared plan. With improved communication between their providers, patients receive smoother, more coordinated care without having to repeat their story. Patients benefit from greater continuity of care, improved trust, and one fully integrated circle of caregivers.
